Domestic certificate of origin Vietnam can find information?

I do SOHO, goods with invoice are paid to export to Vietnam. After the goods arrive at the port, the customer requests to provide form e certificate of origin. As an individual can not apply for form e certificate of origin, I would like to help to handle a form e certificate of origin through an agent. However, the first column of form e certificate of origin handled by the agent can only show the head of the agent company, and the guest requests form E certificate of origin title and bill of lading shipper information should be consistent, so now want to get a copy of high copy form e certificate of origin to provide customers with customs clearance, I do not know whether the domestic certificate of origin can find information in Vietnam.
